Joseph W. Crookshank, Jr., M.D.

Joseph William Crookshank, Jr., M.D., 65, of Lake Charles died at 11:15 AM Tuesday, December 27, in a Lake Charles Hospital.

Dr. Crookshank, one of five generations of physicians, was born in New Orleans and lived in Lake Charles most of his life. He graduated from Lake Charles High School and received his Bachelor's in Accounting and Biology at McNeese before entering Louisiana State University School of Medicine in New Orleans. He returned to Lake Charles as a Board Certified family practice physician and later became Board Certified in emergency room medicine practicing in Southwest Louisiana for twenty-five years.

Dr. Crookshank was a member of the Louisiana Association of Family Practice Physicians as well as a member of the American Academy of Family Practice. He was an avid outdoorsman who loved hunting and fishing. Dr. Crookshank was a member of Our Lady Queen of Heaven Catholic Church.

His father Dr. J. W. Crookshank, Sr. and a grandchild Dylon Bellow preceded him in death.

He is survived by his wife, Drynda Hernandez Crookshank; one son, Joseph William Crookshank, III, M.D.; a daughter, Mandy Louise Crookshank Bellow; his mother, Mary Elaine Fellows Crookshank; two brothers, John Paul Crookshank and Herbert Frank Crookshank; two sisters, Mary Crookshank Reinecke and Lisa Crookshank Henry. He is also survived by two grandchildren, Robyn and Marin Bellow.

A gathering of friends and family will be from 8:00 AM until 9:45 AM on Saturday, December 31, at Johnson Funeral Home. Funeral services will be at 10:00 AM Saturday at Our Lady Queen of Heaven Catholic Church. Monsignor James Gaddy will officiate. Interment of Dr. Crookshank's cremated remains will be at a later date.
